cobol: Integrate PICTURE automaton into parser.

Technical change only.  Remove leftover code and rearrange for
clarity. Use usual compiler include files. Prepare to replace parsing
of alphanumeric-edited and numeric-edited.

This commit also commences new whitespace policy: no committed file
will have trailing whitespace.
